The designated operator failed to document all the alarms from the attached alarm history on the <br />2011 designated operator monthly inspection reports and failed to check that they were responded to <br />appropriately. The missing alarms include: <br />L2- 87 STP sump annular sensor - fuel alarm - 8-12-11 <br />L5- 91 STP sump sensor - fuel alarm - 6-15-11 <br />L6- 91 STP sump annular sensor - fuel alarm - 11-8-11 <br />L8- 91 fill sump annular sensor - fuel alarm - 12-22-11 <br />L14 - vent sump annular sensor - fuel alarm - 1-22-12 <br />L16 - dispenser 1/2 sump annular sensor - fuel alarm - 9-12-11, 11-14-11 <br />L20 - dispenser 5/6 sump annular sensor - fuel alarm - 10/11/11 <br />S1 - 87 product vacuum sensor - no vacuum alarm - 12/20/11, 1/1/12 <br />S2 - 87 tank annular vacuum sensor - no vacuum alarm - 6/23/11 <br />S4 - 91 tank annular vacuum sensor - no vacuum alarm - 4/5/11 <br />During the monthly inspection, the designated operator shall review the alarm history for the previous <br />month, check that each alarm was documented and responded to appropriately, and attach a copy of <br />the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required <br />information on the reports. <br />305, 405. The 87 product fill sump liquid sensor failed when tested. The continuous monitoring system <br />shall be maintained to detect the entry of the liquid or vapor phase of the hazardous substance stored in <br />the primary containment into the secondary containment and to detect water intrusion into the secondary <br />containment. Correct immediately by having a properly licensed, trained, and certified contractor <br />replace the failed component with a functional component (LG 113-x listed, if applicable) and obtain a <br />permit within one business day from the EHD.
