6 t9tavZo <br />MAY 2 4 1989 <br />LODI MEMORIAL HOSPITAL May 1989 <br />Engineering Department WiRONMENTAL HEALTH <br />PERMIT / SERVICES <br />1,000 GALLON GASOLINE TANK <br />MONITORING SYSTEM AND RESPONSE PLAN <br />POLICY: It is the policy of Lodi Memorial Hospital to monitor and control the storage of <br />gasoline in an environmentally safe manner. <br />PROCEDURE:The monitoring of this tank shall include: <br />1 . Annual precision tank test on tank and piping which will be performed during the month <br />of February. <br />2. Tank gauging performed as follows: <br />a. «©e tank will be secured to prevent unauthorized inputs and withdrawals for at <br />least a 48 hour period. <br />b. The tank shall be secured and the liquid 3©® measurement taken and recorded <br />at 5:00 PM on Friday and again at 8:00 AM on the following Monday, at which <br />time the tank will be released for use. <br />C. If liquid level varies more than 1 % of the an volume or 5 gallons, whichever is <br />less, between measurements, an unauthorized release shall be assumed to have <br />occurred and shall be reported to the local agency within 24 hours. <br />
