c) All heavy equipment and service vehicles should be checked at the beginning of <br /> each shift to ensure all parts, equipment and accessories are in good working <br /> condition and free of damage. <br /> d) All heavy equipment and service vehicles on the site must conform with the <br /> applicable inspection codes. <br /> e) Keep a safe distance from moving backhoes, backhoe buckets and other heavy <br /> equipment. <br /> f) All mobile equipment must be equipped with seat belts which must be worn by the <br /> operator at all times, and have approved roll over devices. <br /> g) All job site vehicles will be equipped with a service brake, parking brake and emergency <br /> brake system. <br /> B. In the event of open excavations, which is a hazard to on-site workers, the following policy <br /> regarding excavations will be followed: <br /> a) Entry into excavations greater than 5 feet deep is prohibited, unless the requirements of <br /> TS CCR 1541 are met. Do not stand next to unguarded excavations or trenches. <br /> b) Hazards associated with general construction may occur during the course of <br /> construction. Personnel should be alert and prevent as well to avoid these hazards, <br /> c) Fence and or barricades should be continuously maintained and inspected to assure a <br /> proper buffer between the work area and pedestrians/automobiles is kept. <br /> d) Minimizing the number of personal and/or equipment on site not relative to the tank <br /> preparations and tank entry procedures. <br /> C. During general construction activities, there is also a potential for general (construction type) <br /> safety hazards, This plan does not address general safety in detail. Personnel will <br /> cooperate in being courteous, careful, alert, and thoughtful of the outlined safety procedures. <br /> Common sense should be used in actions and in considering probable consequences. <br /> D. Fires may occur from sources of ignition. <br /> a) Procedures during aeration or preparation for tank entry. <br /> 1. All reasonably movable combustible materials should be removed from the <br /> work area. <br /> 2. All sources of static electricity should be mitigated. <br /> 3. A fire extinguisher having a minimum rating of 2 A, 10 BC should be maintained <br /> nearby, <br /> 4. Operations of all motorized equipment should be kept clear and/or minimized. <br />
