The permit has <br /> been approved with the following conditions. <br /> Conditions of approval: <br /> 1. Contact the local fire department, local planning or building department and the San <br /> Joaquin Valley Air Pollution Control District for any applicable permits. <br /> 2. Any changes shall be submitted to the San Joaquin County Environmental Health <br /> Department(EHD)for approval prior to commencing work.All new addenda supercede <br /> the previous ones. <br /> 3. Before the alarm history is cleared by the"cold start", a current printout must be <br /> created and available for EHD inspection. Inspection may occur during the <br /> functionality test. <br /> 4. Contact Garrett Backus(209-468-2986)to arrange the following inspection(s)(a 48-hour <br /> notice is required): <br /> a. The EHD must witness the testing of the functionality of the monitoring system <br /> to ensure that monitoring has been restored to meet the requirements of <br /> California Code of Regulations Title 23 and the Health& Safety Code. <br /> 5. If concrete is being broken near the UST,pipeline and tank integrity testing must be <br /> performed by a SWRCB licensed tank tester in accordance with the manufacturer's <br /> guidelines. Results must be received by the EHD within seven days of returning the <br /> facility to operating condition. <br /> 6. An"Underground Storage Tank Certification of Installation/Modification" form(form C) <br /> shall be submitted by the owner or the owners'agent [pursuant to(CCR)2635(4)]before <br /> this upgrade will be considered complete by EHD. <br /> 7. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL,ULC,ANSI,API,ASTM,etc. See California Code of <br /> Regulations(CCR),Title 23 Chapter 16 Table B for complete list). The component must <br /> bear the appropriate mark as required by the independent testing organization. <br /> Page 1 of 2 <br />
