Please be advised that the following issues have been addressed: <br /> The monitoring plan and map on site is not current and approved by the Environmental <br /> Health Department (EHD). The Monitoring plan and map do not include the new vapor <br /> condensate trap high level liquid sensor or the annular space liquid sensor. List all liquid <br /> sensors with model numbers and location in the comments section of the monitoring plan or <br /> on the site map. Complete and submit a copy of the monitoring plan and map for approval <br /> to the EHD by June 17,2012. An approved copy will be returned for your records. The update <br /> of the Monitoring Response Plan was assigned to BP vendor, Belshire Environmental Services, and <br /> completed on June 6, 2012. A copy is attached for your records and the approved version will be <br /> placed on site once received back from the EHD. <br /> Secondary Containment testing for the old vapor condensate trap sump was scheduled to be <br /> performed on August 5,2011 by Tank Tek and a test report has not been submitted to the <br /> EHD.This testing was part of the required triennial testing for the facility. A copy of the test <br /> must be submitted with 30 days of the test. By June 17,2012,submit a copy of the test <br /> report to the EHD or explain why the testing was not done. The testing of the condensate trap <br /> was assigned to BP vendor, Tank Tek, and completed on September 2, 2011. A copy is attached <br /> for your records. <br />
