The following is an itemized list of underground storage tank violations that have not been addressed for LOWER SACRAMENTO CHEVRON* as of December 30, 2014. Open violations from October 16, 2014 inspection Violation#106 -Monitoring and response plans not current or approved by the EHD. The monitoring plan is not current and/or not approved by the EHD. The monitoring plan information for the tanks wit ID#"s TA0508375, TA0508376 and TA0508377 is not complete or is incorrect. The tank with ID#TA0508375 -Section for"Tank Monitoring is Performed using the following Methods" is incomplete. -Section for" Pipe Monitoring is Performed Using the following Methods" does not indicate if the line leak detector test is mechanical or electrical. -Section for" Under Dispenser Containment(UDC) Monitoring" is incomplete. -Section for" Periodic System Testing" is incorrect. -Section for" Recordkeeping" is incorrect. -Section for"Training"does not indicate the training done at the facility. The tank with ID#TA0508376 -Section for"Tank Monitoring is Performed using the following Methods" is incomplete. -Section for" Pipe Monitoring is Performed Using the following Methods" does not indicate if the line leak detector test is mechanical or electrical. -Section for" Under Dispenser Containment(UDC) Monitoring" is incomplete. -Section for" Periodic System Testing" is incorrect. -Section for"Training" is incorrect, operating manuals needs to be checked as yes. The tank with ID#TA0508377 -Section for"Tank Monitoring is Performed using the following Methods" is incomplete. -Section for" Pipe Monitoring is Performed Using the following Methods" is incomplete. -Section for" Under Dispenser Containment(UDC) Monitoring" is incomplete. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, make the necessary changes, and submit for review by the EHD. The response plan is not current and/( not approved by the EHD. The response plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a minor violation. Violation#107-Plot plan/site map not submitted or failed to completely show where monitoring is performed. An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a minor violation. Violation#201 -Failed to maintain alarm logs and/or records of follow up actions. Maintenance and monitoring records for the last three years were not found on site. A D.O report states that a technician was notified for a December 17, 2013 Gross Line Fail Alarm, but there was no invoice or other paperwork indicating how the alarm was addressed. These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a repeat violation, Class II. Violation#204-Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessibl on site at all times. Immediately locate and retain a copy on site. This is a minor violation. Page 2 of 3
