The following is an itemized list of underground storage tank violations that have not <br /> been addressed for LOWER SACRAMENTO CHEVRON* as of December 30, 2014. <br /> Open violations from October 16, 2014 inspection <br /> Violation #207-Designated operator did not inspect all required items and/or the inspection reports not <br /> completed. <br /> The designated operator failed to document all the alarms from the attached alarm history on the August 2014 or the <br /> September 2014 designated operator monthly inspection report and failed to check that they were responded to <br /> appropriately. The missing alarms include: Q2-Q3 alarms for August 8, 2014, August 13, 2014 and August 14, 2014 <br /> During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br /> that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br /> documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br /> monthly inspections at this facility are including all of the required information on the reports. This is a minor violatior <br /> Violation#306 -Monitoring equipment is not installed or maintained to detect a leak at the earliest <br /> opportunity. <br /> The 89 turbine sump sensor was not at the lowest point of the sump and not located to detect a leak at the earliest <br /> opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity <br /> The sensor was relocated to the lowest point of the sump by the service technician. Ensure that all monitoring <br /> equipment is maintained to detect a leak at the earliest opportunity. This is a Class II violation. <br /> Violation#319 -Water in secondary containment not removed,analyzed, and properly disposed of(pre4ul <br /> 2003). <br /> Liquid was observed in the 91 turbine sump on the west side of the sump. If water could enter into the secondary <br /> containment by precipitation or infiltration, it must be removed and disposed of properly. Immediately remove this <br /> liquid, make a hazardous waste determination per Title 22 hazardous waste regulations, and manage it accordingly. <br /> Ensure that the 91 turbine sump is maintained free of liquid. This is a Clays II violation. <br /> Page 3 of 3 <br />
