0 <br />C <br />C <br />UNOCAL will notify the appropriate State and Local agencies unless the situation <br />requires urgent immediate response by the agencies, in which case the DEALER should <br />notify these agencies: <br />1. LOCAL AGENCY: Office of Emergency Services <br />PHONE NUMBER: 209-468-3969 <br />2. CALIFORNIA OFFICE OF EMERGENCY SERVICES: (800)852-7550 (24 HOURS) <br />3. SAN JOAQUIN COUNTY HEALTH SERVICES: (209)468-3406 <br />8. Dealer should attempt to isolate leak location by visual inspection. <br />9. UNOCAL Representative, Matt Fischer, will coordinate with UNOCAL Maintenance and <br />Construction crews as to corrective actions need to be taken beyond the Dealer's <br />capabilities. UNOCAL Maintenance and Construction crews will file whatever reports <br />need to be filed with local and state agencies mentioned in Item 7 and send a copy to <br />the station for the Dealer's file. <br />A COPY OF THIS PAGE AND PRECEEDING PAGE MUST BE POSTED <br />CONSPICUOUSLY ON SITE. <br />in case of a leak or spill, the dealer must complete Form E to attach to Form A, and a copy <br />of Form F must be sent to the UNOCAL Representative within 24 hours. The Dealer must <br />also notify his representative by phone (and/or call the UNOCAL Emergency Phone after <br />hours). <br />The Dealer must post a copy of Page 5 at a conspicuous location in the cashiers area. <br />The Dealer must keep a copy of Form H to document the training received by the station's <br />employees. <br />COPIES OF ALL FORMS MAILED OUT BY THE DEALER OR HIS EMPLOYEES MUST BE KEPT <br />ON FILE AT THE STATION. <br />A <br />
