Kettleman Lane, <br />Lodi. <br />I have reviewed the information you submitted in response to the March 23, 1998 <br />memo. Please submit the following information. <br />I. You still need to identify the site EPA # on page three of the application. <br />2. Please be aware that a fire permit will be required from the city of Lodi before the <br />tank can be removed. <br />;. The contractor still needs to submit a site health and safety plan, <br />4.
