Kettleman Lane, Lodi. <br />I have reviewed the removal application for the above referenced facility. The following items need to be <br />submitted to this office before the permit can be issued: <br />✓ 1. You need to submit all information in triplicate. I am returning the original application to you so it can <br />be completed and returned to this office with two additional copies. <br />You need to identify an EPA site # on page three. <br />�3. You need to identify the project contact and telephone # on page three. <br />0 You need to complete the fire district, laboratory name (county & phone #), and sampling firm (phone <br />#) on page three. :d" t«< <br />5. The contractor needs to submit a site health and safety plan for this job. <br />6. Page four of the application identifies that the tank and piping will be decontaminated prior to removal <br />but #7, e, does not identify a rinsate hauler and permitted TSDF. Please complete. <br />7.
