Violations 105 and 106 have been rescinded. The required <br /> information has already been entered into the California Electronic Reporting System. This report <br /> supersedes the initial report provided on 11-20-13. <br /> On site to conduct the routine inspection and witness the annual monitoring system certification testing. <br /> 202. Current approved copies of the monitoring and response plans were not found on site. The <br /> monitoring and response plans must be current and approved by the EHD. A copy of the approved <br /> monitoring plan and response plan were provided to the site operator during the inspection. <br /> 201. Test records for the most recent monitoring system certification and secondary containment testing <br /> were not on site. Maintenance records and a facility alarm log for the 8-25-13, 7-16-13, and 5-15-13 <br /> diesel sump sensor alarms were not found on site. These records shall be kept on site for at least 3 <br /> years. Immediately locate and maintain the test records, maintenance records, and a facility alarm log <br /> on site and submit copies to the EHD by December 20, 2013. <br /> 204. The designated operator failed to document all the alarms from the attached alarm history on the <br /> September 2013, July 2013, and May 2013 designated operator monthly inspection reports and failed to <br /> check that they were responded to appropriately. The missing alarms include: 8-25-13, 7-16-13, and <br /> 5-15-13 diesel sump alarms. During the monthly inspection, the designated operator shall review the <br /> alarm history for the previous month, check that each alarm was documented and responded to <br /> appropriately, and attach a copy of the alarm history with documentation taken in response to any <br /> alarms to the monthly report.
