The designated operator employee training for Zulfiqar Khan was last done on 4-24-12 and Jaspal <br /> Koont was last done on 11-2-12. The designated operator employee training is no longer current. The <br /> designated operator shall train facility employees for which he or she is responsible in the proper <br /> operation and maintenance of the UST system once every 12 months. The training shall include, but is <br /> not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br /> Ensure that employees have been trained by the designated operator and maintain the list on site. <br /> During the inspection, Jaspal Koont and Zulfiqar Khan were trained by the facility designated operator. <br /> 311. The 87 line leak detector failed to detect a leak when tested. All line leak detectors shall be <br /> capable of detecting a 3-gallon per hour leak at 10 psi. The leak detector was replaced and retested <br /> during the inspection. A "UST Retrofit Verification with Inspector Already Onsite" form has been <br /> completed and provided to the operator and contractor. <br /> 313. The 87 direct-bury spill container failed when tested. All spill containers shall have a minimum <br /> capacity of five gallons and be capable of containing a spill or overfill until it is detected or cleaned up. <br /> During the inspection, the technician tightened up the fill adaptor and retested the spill container to pass. <br /> Complete and submit a copy of the Return to Compliance Certification form with a statement of how <br /> each violation was corrected, including supporting documentation, by December 20, 2013. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO EINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> Aris Cacapit (209) 468-3453 6-v�w , <br /> EHD 23-02-003 Rev 04/19/12 CONTINUATION FORM <br />
