However on the UST report dated 12,122/09 (see. <br />attached) our Designated Operator noted That the alarms on I 1 i30 & 12106 weren't <br />recorded on alarm sheet, nor u -m any exp1miadon recorded or reported, <br />Solution: All alarms must be written on alarm log that is to be kept at-K)ve monitoring <br />panel. Site must note date, type of alarm and action tAert W correct it. If alarm shuts <br />itself off, or clears after a few hours, that too MUST be racordcd. For lack of <br />documentation for this issue, keeping complete records <br />(including: DO reports, alarm history print out, and all invoices) onsite M If eliminate a <br />need to "hunt" for these solutions - <br />Action: )Ve have attached the copy orDecernber's UST' report to Show dust. our DO did <br />in fact note the alarms, and as result lack of correct action taken. We have not hired new <br />DO's, however we do rotate the DO's if needed. A sbect was sent out in December of <br />21009 listing current DO's wit1h, ou" company and their Certification numbers and <br />expiration dates. 1.7his form Was signed and sent flack by site on December 27, 2009. (see <br />attached) <br />Solution: When we send oltt okhmer Statements of DO, a copy should be kept onsite and <br />the site needs to insure the county haws a cow also. <br />Action: We have attached the signed document.
