Since site did not <br />documm alann or action taken, or DO Only had site's word to go off of <br />Solution: All alarms must be written on alarm log that is 1.0 be kept abLive monitoring <br />panel, Site must note date, type of alarm and action taken to correct it, if alarm shuts <br />itself off, oi clears aftcr a few hours, that too MUST be recorded. For lack of <br />documentation for this issue, keeping complete records <br />(including: DO reports, alarm histor,%, print out, and. all invoices) ousitc 1,vjlj eliminate <br />ticed to "hunt" for these solutions. <br />Actiou: We have attached the copy of June's UST report to show that our DO did in fact <br />note the alarms. We have also attached all' a!am history- report as well. We have alsoattached a COPY orthe Mwnitoflng system <br />alarm log, which will prevent the Violation t'ot noir-recorded alarms and actions.
