Lockeford St. Lodi, CA 95240 <br />Program: UST <br />SUMMARY OF VIOLATIONS <br />CLASS I, CLASS 11, or MINOR -Notice to Comply) <br />On site to conduct a routine inspection and witness the monitoring system certification testing. <br />4. Financial responsibility documents have not been submitted to the EHD. Current financial <br />responsibility documents are required to be submitted annually. Jaspal Koont corrected this during the <br />inspection by providing updated financial responsibility documents. Continue to ensure that the required <br />financial responsibility documents are completed and submitted annually. Repeat Violation. <br />7. Maintenance records for the 91 and diesel stp sump alarms and the 12-6-09 alarm were not found on <br />site. These records shall be kept on site for at least 3 years. Immediately locate and maintain the <br />maintenance records for these alarms on site and submit copies to the EHD by 12-23-10. Repeat <br />Violation. <br />11. The 87, 91, and diesel sump sensors were laying down on their sides and not located to detect a <br />leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at <br />the earliest possible opportunity. The technician corrected this during the inspection by position the <br />sensors upright at the lowest part of the sump. Continue to ensure that the sensors are properly <br />positioned upright at the lowest point of the sump. <br />23a. A new designated operator was hired on 2-10-10 and 9-15-10 and notification was not provided to <br />the EHD within 30 days of the change. Any changes shall be reported to the EHD with 30 days. Submit <br />notification to the EHD identifying all the designated operators for this facility immediately. Repeat <br />Violation. <br />23c. The designated operator failed to document all the alarms for the previous month and check that <br />they were .responded to appropriately on the 6-28-10 and 11-30-09 designated operator monthly <br />inspection report. The missing alarms include:91 and diesel stp sump alarms on 6-15-10 and al! sensor <br />alarms on 11-18-08.
