Lockeford St. Lodi, CA 95240 Program: UST SUMMARY OF VIOLATIONS CLASS 1, CLASS 11, or MINOR -Notice to Comply) 23c. The designated operator failed to check and note the correct test dates for the Monitoring Certification, Leak Detector, and Spill Bucket tests on the 11-30-09 designated operator monthly inspection report. The 11-30-09 D.O. report listed the Monitoring Certification, Leak Detector, and Spill Bucket tests as 12-4-08, but it should be listed as 11-18-08. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Repeat Violation. 23c. The designated operator failed to verify that all employees have been trained during the May through October 2010 monthly inspections. Employee Zulfiqar Khan was hired May 2010 per Jaspal Koont and has not yet been trained by the D.O. During the first part of the inspection Zulfiqar Khan was working by himself. Jaspal Koont is the only D.O. trained employee at this time. During the monthly inspection, the designated operator shall verify that all facility employees have been trained. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Repeat Violation. 23d. The designated operator employee training for Zulfiqar Khan was not performed. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD by 12-23-10. Complete and submit a copy of the Return to Compliance Certification form with a statement of how each violation was corrected, including supporting documentation, by 12-23-10. ALL EHD STAFF 71ME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE ($122).
