A new designated operator,Alexander Tate was hired on 11-19-14 and notification was not provided to the EHD within 30 days of the change. Any changes shall be reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a repeat violation, Class II. 204 CCR 2712(1) Current monitoring plan approved by the EHD not found on site. An approved copy of the current monitoring plan was not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a repeat violation, Class II. 206 HSC 25294, 25295 Failed to record or report unauthorized release in appropriate time frame. A small amount of diesel fuel was found in the#3/4 under dispenser containment sump and diesel turbine sump, indicating a leak in the primary containment system. Diesel fuel was observed to have leaked from the diesel fuel filter in dispenser#3/4. This release of diesel fuel was not recorded nor cleaned up yet. Any release from the primary containment that does not escape the secondary containment and was cleaned up within eight hours of being detected shall be recorded with the following information: 1. Operator's name and phone number 2. List of types, quantities, and concentrations of hazardous substances released 3. Description of actions taken to control and clean the release 4. Method and location of disposal of the released substances 5. Description of actions taken to repair and prevent future releases 6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary containment Immediately record this release with the information required, and maintain it on site with the facility's monitoring records. Submit a copy of the recordable release statement to the EHD. This is a repeat violation, Class ll. 306 CCR 2630(d) Monitoring equipment is not installed or maintained to detect a leak at the earliest opportunity. The 91 and 87 turbine sump sensor were laying on their side and not located to detect a leak at the earliest opportunity. Monitoring equipment shall be maintained to be able to detect a leak at the earliest possible opportunity. The sensor was relocated to the lowest point of the sump in an upright position by the service technician. Ensure that all monitoring equipment is maintained to detect a leak at the earliest opportunity. This is a repeat violation, Class II.
