The following sections in the monitoring plan <br /> submitted to the California Environmental Reporting System for the 87, 91, and diesel tanks were left blank: Pipe <br /> Monitoring is Performed Using the Following Method(s), UDC Monitoring Stops Flow of Product at Dispenser in the <br /> Under Dispenser Containment(UDC) Monitoring was not marked no, Designated Operator Training was not marked <br /> yes. The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately <br /> log into CERS, make the necessary changes, and submit for review by the EHD. This is a Class II violation. <br /> The response plan submitted to CERS is not current and/or not approved by the EHD. The response plan must be <br /> uploaded to the California Environmental Reporting System (CERS). Immediately log into CERS, upload the <br /> completed form, and submit for review by the EHD. Ensure a copy is available for inspection at any time. This is a <br /> Class II violation. <br /> 107 CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br /> An accurate UST Monitoring Site Plan was not submitted in CERS. A site plan must be submitted identifying the <br /> locations where monitoring will be performed. Immediately log into the California Environmental Reporting System <br /> (CERS) at http://cers.calepa.ca.gov/and upload a copy of the UST Monitoring Site Plan. This is a Class II violation. <br /> 113 CCR 2638(d) Failed to submit monitoring system certification test report within 30 days. <br /> Annual monitoring system certification, leak detector testing, and spill container testing was performed on 11-20-13 and <br /> a test report was submitted on 2-14-14, approximately two months late. A copy of the test report must be submitted <br /> within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports are <br /> performed in a timely manner. This is a Class II violation. <br /> 201 CCR 2712(b) Failed to maintain alarm logs and/or records of follow up actions. <br /> Maintenance and monitoring records for the last three years were not found on site. On 5-25-14, a diesel sump alarm <br /> occurred. The designated operator and the facility operator did not document this alarm. These records shall be <br /> maintained on site for at least three years. Monitoring records include: (1) date and time of all monitoring or sampling; <br /> (2) monitoring equipment calibration and maintenance records; (3) results of any visual observations; (4) results of <br /> sample analysis performed a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground <br /> water elevations, or other test results; (6) results of inventory readings and reconciliations. Immediately locate and <br /> maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD.
