The designated operator failed to document all the alarms from the attached alarm history on the 6-4-14 designated operator monthly inspection report and failed to check that they were responded to appropriately. The missing alarms include: 5-25-14 diesel sump alarm. The 9-10-14 designated operator monthly inspection report did not have a copy of the alarm history attached. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. The designated operator failed to check and note the correct test dates for the secondary containment test on the 2-5-14 designated operator monthly inspection report. The secondary containment test date was listed as 2-5. It should be listed as 2-5-14. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. Overall Inspection Comments: On site to conduct a routine inspection and witness the annual monitoring system certification testing. This completed report supersedes the checklist left with the operator on 11-19-14. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 12-20-14. Consent to perform the inspection, take photos and collect samples was given by employee, Zulfiqar Khan. This facility's designated operator is James Williams (ICC expiration date: 6-4-15). The service technician was Alexander Tate (ICC expiration date: 3-19-15), who had the following manufacturer's certifications: Incon EXP. DATE 8-12-15 and VMI EXP. DATE 1-29-15. Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator Compliance.
