The permit has been approved with the following conditions: <br /> 1. Contact the Local Fire Department,Local Planning or Building Department,and the San <br /> Joaquin Valley Air Pollution Control District for any applicable permits and <br /> requirements. <br /> 2. Contact Muniappa Naidu to arrange the following inspections(a 48 hour notice is <br /> required): <br /> a. Verification of monitoring system to ensure it was unaffected by the EVR phase <br /> II upgrade and dispenser replacement. If necessary a functionality test must be <br /> performed to verify monitoring system meets the requirements of California <br /> Code of Regulations Title 23 and the Health& Safety Code. <br /> b. A copy of the alarm history prior to commencement of work and after <br /> completion of work must be available for review at the time of EHD inspection. <br /> 3. Qualified installers must possess or work under the direct and personal supervision of an <br /> individual physically present at the work site,at all times while work is being performed, <br /> who possesses all of the following: <br /> a. One of several permissible valid,current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company),and <br /> b. All valid,current California UST certificates issued by the International Code <br /> Council(ICC),that are applicable to the work being performed, and <br /> c. A valid,current certificate of training from the manufacturer(s)of the <br /> component(s)being tested,repaired, or serviced. <br /> And in addition <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 4. Qualified service technicians must possess or work under the direct and personal <br /> supervision of an individual physically present at the work site who possesses all of the <br /> following: <br /> Page 1 of 2 <br />
