One of several permissible valid,current contractor's licenses issued by the <br /> Contractors State License Board(may be issued to the installation company),and <br /> b. A valid,current California UST Service Technician certificate issued by the <br /> International Code Council(ICC),and <br /> c. A valid,current certificate of training from the manufacturer(s)of the <br /> component(s)being tested,repaired,or serviced,and <br /> d. Each person installing or testing any component must hold a certificate of <br /> training from the manufacturer of that component. <br /> 5. Permit expires 180 days from date of approval. Note: If an extension to the expiration <br /> date is required,a written request must be submitted to this office prior to the expiration <br /> date. <br /> 6. The contractor stated that the conversion adaptor will not be required for the replacement <br /> of the dispenser, if during inspection the size of the under dispenser containment does not <br /> match the dispenser size a conversion adaptor will be required. <br /> 7. All components must be certified by an independent testing organization and labeled as <br /> required by the organization. (UL,ULC,ANSI,API,ASTM,etc. See California Code of <br /> Regulations(CCR),Title 23 Chapter 16 Table B for complete list). The component must <br /> bear the appropriate mark as required by the independent testing organization. <br /> 8. Any changes in information and scope of work must be submitted to EHD for approval <br /> prior to commencing work.
