Current approved copies of the Monitoring and Response Plans were not found on site. The <br />Monitoring and Response Plans must be current and approved by EHD. <br />- Copies made and kept on site. <br />108. Secondary containment was not performed in a timely manner. Secondary containment testing <br />was last performed on 11/08/2008 and the next tri -annual secondary containment testing was due by <br />11/2011. Testing has not been completed and is three months past due. Secondary containment <br />testing is required once every 36 months. Immediately schedule this test and provide 48 hours <br />notification to the EHD. <br />In addition, the last secondary containment testing indicated a failure in the under dispenser containment <br />(UDC) 1/2, 3/4, 5/6, 7/8, the 87 secondary product lines run #2, #3, #4 and 91 secondary line run #1. All <br />secondary containment for the UST system must be tight. Immediately have a properly licensed, trained <br />and certified contractor replace or repair the failed component under permit and inspection of EHD and <br />submit passing results to this office. An permit has been taken out and approved for the repairs under <br />SR0062373 but repair date has not been set. <br />113. Failed to inform EHD of change of designated operators (DO) within 30 days. A designated <br />operator Steve Zwahlen (exp 05/01/12) from Franzen -Hill performed monthly visual inspection reports <br />for this facility for the month of November - December 2010, January - March 2011. Designated operator <br />Chris Deyoung (exp 08/08/13) from Franzen -Hill performed monthly visual inspection reports for this <br />facility for the month of August 2011. Designated operator Tyne Hardeman (exp 11/11/13) from <br />Franzen -Hill performed visual monthly inspection report for the month of December 2011. Any changes <br />in designated operators shall be reported to EHD within 30 days.
