The designated operators (DO) failed to check and note the correct test dates for the annual monitoring system certification (MSC), annual spill containment (SC) test and the tri -annual secondary containment test on several of the designated operator monthly inspection reports. -December 2011, DO Tyne Hardeman documented secondary containment testing as 12/28/2008 -November 2011, DO David Martin documented secondary containment testing as 12/28/2008 -August 2011, Chris Deyoung documented MSC/SC as12/14/2009. Secondary containment testing as "No Recorded" -July 2011, DO James Flowers documented MSC/SC as 04/08/2011. Secondary containment testing as 03/04/2009 -June 2011, DO James Flowers documented MSC/SC as 12/14/2010 (site tested/ CLI PA not notified), secondary containment as "No record" -May & April 2011, DO James Flowers documented MSC/SC as 11/25/2009. Secondary containment testing as "No record" -March, February, January 2011, December 2010, DO Steve Zwahlen documented MSC/SC as 01 /10/11. Secondary containment testing as "No record" -November 2010, DO Steve Zwahlen documented secondary containment testing as "No record" During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. If secondary containment testing has not been done or there was "No record", investigate why the test has not been completed or why there is no test results on site for review. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 206. Designated operator training not current and not performed annually. Last designated operator training for Sherry Blakeney was performed 07/13/10 and Sandra Harp on 10/01/10 and both are over due for their annual refresher. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include but not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. Reminder next training for Casey Avila is due by 09/2012 and Balwinder Singh by 10/2012. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
