A new designated operator began conducting the monthly designated operator inspections in April <br />2011 and notification was not provided to the EHD within 30 days of the change. Any changes shall be <br />reported to the EHD with 30 days. An updated notification was submitted at the time of inspection. <br />REPEAT VIOLATION <br />201. The alarms from February 20, 2011 (unleaded sump) and February 25, 2011 (dispenser 7-8) were <br />not documented in the alarm log. Maintenance records for the response to these alarms were not found <br />on site. These records shall be kept on site for at least 3 years. Immediately locate and maintain the <br />maintenance records on site and submit a statement documenting the corrective actions taken in <br />response to these alarms to the EHD by October 9, 2011. <br />REPEAT VIOLATION <br />204. The designated operator failed to document all the alarms from the attached alarm history on the <br />designated operator monthly inspection reports and failed to check that they were responded to <br />appropriately. The missing alarms include: February 20, 2011 (unleaded sump), January 29, 2010 <br />(dispenser 1/2, 3/4, 5/6), January 28, 2010 (unleaded sump), April 12, 2010 (dispenser 7/8), and <br />December 16, 2009 (dispenser 7/8). During the monthly inspection, the designated operator shall <br />review the alarm history for the previous month, check that each alarm was documented and responded <br />to appropriately, and attach a copy of the alarm history with documentation taken in response to any <br />alarms to the monthly report.
