The designated operator failed to check and note the correct test dates for the monitoring system certification, secondary containment testing, and spill bucket testing on the January, February, March, April, May, June, and July designated operator monthly inspection reports. The January through June 2011 inspection reports state "no record" under the secondary containment test date. For the monitoring system certification and spill bucket test dates, the April and May 2011 inspection reports list November 25, 2009, the June 2011 report lists December 14, 2010, and the July 2011 report lists April 8, 2011. The monitoring system certification was conducted on January 10, 2011. The secondary containment testing was conducted on November 18, 2011, but did not pass. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 205. The August 2011 designated operator monthly inspection report was not found on site. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Locate and ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. Submit copies to the EHD by October 9, 2011. REPEAT VIOLATION 206. The designated operator employee training for Balwinder Singh, Kim Cruz, and Sharon Winter could not be located on site. The designated operator employee training for Sherry Blakeney is not current (last conducted July 13, 2010). Mr. Balwinder was the only employee on site at the time of inspection. The monthly designated operator reports state that training was conducted for employees on March 2, 2011. No specific employees were listed; no 2011 training records were located on site. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms At least one facility employee present during operating hours shall have current training. Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD by October 9, 2011. REPEAT VIOLATION ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
