9 <br />The following is an itemized list of underground storage tank violations haveto, �s <br />been addressed for ESCALON MINI MART as of March 29, 2 V 0 <br />Open violations from December 13, 2016 inspection <br />Violation #103 - Current financial responsibility documents not submitted. <br />Financial responsibility documents have not been submitted to the EHD. Current financial responsibility documents <br />are required to be submitted annually. Immediately log into the California Environmental Reporting System (CERS) <br />at http-.Hcers.calepa.ca.gov/, and upload the required documents. <br />Violation #107 - Plot plan/site map not submitted or failed to completely show where monitoring is <br />performed. <br />An accurate UST Monitoring Site Plan was not submitted. The site map does not show the location of monitoring <br />equipment. A site plan must be submitted identifying the locations where monitoring will be performed. Immediately <br />log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/ and upload a copy of <br />the UST Monitoring Site Plan. <br />Violation #115 - Failed to inform the EHD of designated operator identification or change within 30 days. <br />A new designated operator was hired on since 2015 and notification was not provided to the EHD within 30 days of <br />the change. Shawn Rodriquez, Jose Ochoa and Jason Weaver have conducted designated operator inspections <br />and notification was not given to the EHD. Any changes shall be reported to the EHD within 30 days. Immediately <br />log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload the <br />notification identifying all the designated operators for this facility. <br />Violation #121 - Failed to submit a written response within 30 days of receiving an inspection report. <br />An inspection was last done on January 11, 2016 and an inspection report was issued identifying information to be <br />submitted to bring this site into compliance. This information was required to be submitted by February 11, <br />2016.This information has not been received resulting in a non-compliant status for this facility. An operator that <br />receives an inspection report shall have 30 days to submit a written response that includes a statement <br />documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written <br />response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection <br />report. Violations not corrected: <br />- HSC 25292.2 Current financial responsibility documents not submitted <br />- CCR 2632(d)(1)(C) Plot plan/site map not submitted or failed to completely show where monitoring is performed. <br />- CCR 27(a) Failed to inform the EHD of a change of designated operator within 30 days. <br />- CCR 2712(b) Failed to maintain alarm logs and/or records of follow up action. <br />- CCR 2715 Designated operator did not inspect all required items and/or the inspection reports not complete. <br />- CCR 2715(f) Designated operator employee training not performed or log kept. <br />Violation #201 - Failed to maintain alarm logs and/or records of follow up actions. <br />Maintenance and monitoring records for the last three years were not found on site. The January 2016 monitoring <br />system certification test results were not found on site. These records shall be maintained on site for at least three <br />years. Monitoring records include: (1) date and time of all monitoring or sampling; (2) monitoring equipment <br />calibration and maintenance records; (3) results of any visual observations; (4) results of sample analysis performed <br />a lab or in the field; (5) logs of all readings of gauges or other monitoring equipment, ground water elevations, or <br />other test results; (6) results of inventory readings and reconciliations. Immediately locate and maintain all <br />maintenance and monitoring records for the last three years on site and submit copies to the EHD. <br />Violation #309 - Monitoring in sump / UDC failed to activate an audible and visual alarm or stop the flow <br />product. <br />The 91 STP sump sensor failed to activate an audible and visual alarm when tested to ensure continuous <br />monitoring of the tank system. All monitoring equipment shall be maintained to activate an audible and visual alarm <br />or stop the flow of product at the dispenser when it detects a leak. Correct immediately by having a properly <br />licensed, trained, and certified contractor replace the failed component with a functional component (LG 113-x <br />listed, if applicable) and obtain a permit within one business day from the EHD. If the failed component can't be <br />replaced immediately, there is a possibility that the 91 STP UST system may be red tagged to prevent fuel inputs. <br />Open violations from January 11, 2016 inspection <br />Page 1 of 2 <br />
