The following is an itemized list of underground storage tank violatio- , adhav <br />been addressed for ESCALON MINI MART as of March 29, '17. G" <br />Open violations from January 11, 2016 inspection <br />Violation #103 - Current financial responsibility documents not submitted. <br />Financial responsibility documents have not been submitted to the EHD. The submitted form is dated 11-5-2014. <br />Current financial responsibility documents are required to be submitted annually. Immediately log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, and upload the required documents. <br />Violation #107 - Plot plan/site map not submitted or failed to completely show where monitoring is <br />performed. <br />An accurate UST Monitoring Site Plan was not submitted. A site plan must be submitted identifying the locations <br />where monitoring will be performed. The submitted map lacks the legend for the utilized symbols identifying <br />monitoring locations. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/ and upload a copy of the UST Monitoring Site Plan. <br />Violation #115 - Failed to inform the EHD of a change of designated operator within 30 days. <br />A new designated operator was hired at least a year ago and notification was not provided to the EHD within 30 <br />days of the change. Jacob Weaver, Gabriel Vonegas, Jpse Ochoa, and James Flowers have conducted the <br />designated operator inspections in 2015 and the notification was not provided to the EHD. Any changes shall be <br />reported to the EHD within 30 days. Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. <br />Violation #201 - Failed to maintain alarm logs and/or records of follow up actions. <br />Maintenance and monitoring records for the last three years were not found on site. The onsite alarm logs didn't <br />document the following alarms: Dec 11, 2015 (91 STP sump). December 31, 2015 (87 STP sump), and December <br />31, 2015 (annular sensor). These records shall be maintained on site for at least three years. Monitoring records <br />include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance <br />records; (3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs <br />of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br />inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for <br />the last three years on site and submit copies to the EHD. Submit proof of correction to the EHD. <br />Violation #209 - Designated operator did not inspect all required items and/or the inspection reports not <br />completed. <br />The designated operator failed to document all the alarms from the attached alarm history on the November 24, <br />2015 designated operator monthly inspection report and failed to check that they were responded to appropriately; <br />the documentation didn't indicate the date of the alarm and only listed that the alarm was for the premium sump. <br />During the monthly inspection, the designated operator shall review the alarm history for the previous month, check <br />that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with <br />documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing <br />monthly inspections at this facility are including all of the required information on the reports. <br />Violation #211 - Designated operator employee training not performed or log not kept. <br />The designated operator employee training log doesn't appear to have been updated, according to Mr. Singh the <br />training was provided. The designated operator shall train facility employees for which he or she is responsible in <br />the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is <br />not limited to: <br />1. Operation of the UST system in a manner consistent with the facility's best management practices <br />2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br />3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br />4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of <br />the training records to the EHD. <br />Page 2 of 2 <br />
