Louise Ave., Lathrop, Ca. <br /> After reviewing your plan and application for permit to close and to install underground <br /> storage tanks (UST) at the above facility,I observed the following deficiencies: <br /> Tank removal <br /> 1. Site safety officer. <br /> 2. Site EPA number; per application, site EPA# is CAR 000000265. Is this correct? <br /> CAR does not exist in EPA number. <br /> 3. Information on state-approved laboratory and third parry sampling firm. See page 3 <br /> of application. <br /> 4. Describe the method to be used in decontaminating the tanks and piping. "Water <br /> rinsate" is not an acceptable method. See item 7c of application. <br /> 5. Permitted disposal site for rinsate. Ramos Environmental is not a permitted disposal <br /> facility. See item 7e of application. <br /> 6. The destination of the excavated stockpile. West Coast is not a disposal site or <br /> permitted to accept contaminated stockpile. See item 1 lb of application. <br /> 7. Contaminated soil hazardous waste hauler and hauler registration number. West <br /> Coast Equipment is not a registered hazardous waste hauler. See item 11 c of <br /> application. <br /> Tank Installation <br /> 1. The application for permit to install UST is outdated. Enclosed is a revised and <br /> currently used application. Please complete and submit it to me in triplicate. <br /> 2. Site EPA number;per application, site EPA# is CAR 000000265. Is this correct? <br /> CAR does not exist in EPA number. <br /> 3. The proposed location of the monitoring panel. <br /> 4.
