Check list provided only. Written report & photos provided on March 27, 2012. Addendum 04/04/13- removing violation 999. HSC 25185(c)(3) 104. Financial responsibility documents have not been submitted to EHD for 2011, 2012 and 2013 year. Current financial responsibility documents are required to be submitted annually via fax or postal mail. Complete and submit a copy of the financial responsibility documents for 2013 year to EHD within 30 days of this report. Submit an explanation how the operator/ owner submits this documents in a timely manner to EHD annually. - Received copy of 2013 on site. 108. Secondary containment testing was last performed on May 28, 2009 and was due by May 28, 2012. Testing has not been completed and is 10 months past due. Designated operator Karen Arnaiz documented that secondary containment "Items Requiring Follow-up Actions: Have SB -989 testing performed" on every monthly report since August 2012 to present. It appears that the designated operator drop off the reports to the site but no facility employees are reviewing the documents. Secondary containment testing is required once every 36 months. Immediately schedule this test and provide 48 hours notification to EHD. - Phone call & fax from Afforda-test stated test scheduled for March 28, 2013. 206. The designated operator employee training for Sunny Singh, "Jasper", Sandra Perkins and Jagjit Singh was last conducted May 25, 2011. The next annual training was due by May 2012. Instead, the designated operator Karen Arnaiz did not conduct the training until November 8, 2012, approximately six months past due. Ensure next designated operator employee training is conducted on an annual basis. Explain how employee training will be conducted in a timely manner. 109. Annual monitoring system certification, leak detector testing, and spill container testing was performed on July 12, 2012 and a test report has not ever been submitted to the EHD. A copy of the test report must be submitted within 30 days of the tests. Immediately submit a copy of the test report to the EHD. - Corrected on site. A copy was received. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
