The designated operator failed to document all the alarms responses from the attached alarm <br />history on the August 2012 designated operator monthly inspection report and failed to check that they <br />were responded to appropriately. The missing alarm response include: August 08, 2012. Designated <br />Operator Karen Arnaiz had a alarm history print out dated August 7, 2012. The monthly report was dated <br />August 9, 2012. During the monthly inspection, the designated operator shall review the alarm history for <br />the previous month, check that each alarm was documented and responded to appropriately, and attach <br />a copy of the alarm history with documentation taken in response to any alarms to the monthly report. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the <br />required information on the reports. <br />204. The designated operator failed to document all the alarms responses from the attached alarm <br />history on the February and March 2013 designated operator monthly inspection report and failed to <br />check that they were responded to appropriately. The alarm history printout from the Veeder-Root <br />monitoring panel states "Set up data warning June 6, 2008 @ 8:02AM" for all sensors L1 -L10 for both <br />months. It is unclear what happened to the monitoring panel. During today's annual compliance testing, <br />the alarm history report for all sensors L1 -L10 states "Set up data warning March 7, 2013 @ 5:05PM". <br />During the monthly inspection, the designated operator shall review the alarm history for the previous <br />month, check that each alarm was documented and responded to appropriately, and attach a copy of <br />the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that <br />designated operators performing monthly inspections at this facility are including all of the required <br />information on the reports. Submit an explanation for the cause of the Set up data warning along with <br />any work orders and how the Veeder-Root monitoring panel restored its programing. <br />204. The designated operator failed to check and note the correct test dates for the secondary <br />containment testing/ SB -989 on the designated operator monthly inspection reports from November <br />2010 to July 2012 (20 months). No date was documented and the designated operator check marked <br />'NA'. During the monthly inspection, the designated operator shall check that all required testing and <br />maintenance for the UST system have been completed and document the dates they were done. <br />Ensure that designated operators performing monthly inspections at this facility are including all of the <br />required information on the reports.
