According to the designated operator,the UDC has been leaking since it was <br /> discovered during the April 2017 monthly inspection report. Any release from the primary containment that does not <br /> escape the secondary containment and was cleaned up within eight hours of being detected shall be recorded with <br /> the following information: <br /> 1. Operator's name and phone number <br /> 2. List of types,quantities, and concentrations of hazardous substances released <br /> 3. Description of actions taken to control and clean the release <br /> 4. Method and location of disposal of the released substances <br /> 5. Description of actions taken to repair and prevent future releases <br /> 6. Description of method used to reactivate interstitial monitoring after repair or replacement of primary <br /> containment <br /> Immediately record this release, along with the other designated operator's documented releases with the <br /> information required, and maintain it on site with the facility's monitoring records. Submit a copy of the recordable <br /> release statement to the EHD. <br /> This is a minor violation. <br /> 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. <br /> The designated operator(DO)failed to check and note the correct test dates for the secondary containment testing. <br /> The DO incorrectly stated that the latest test occurred on 10/23/2013 for the 11/7/2016, 12/1/2016 and 1/3/2017 <br /> designated operator monthly inspection reports.The correct date is 10/17/2016. During the monthly inspection, the <br /> designated operator shall check that all required testing and maintenance for the UST system have been completed <br /> and document the dates they were done. Ensure that designated operators performing monthly inspections at this <br /> facility are including all of the required information on the reports. <br /> This is a minor violation. <br /> 211 CCR 2715(f) Designated operator employee training not performed or log not kept. <br /> The designated operator employee training for Usha Harit and Hardal Harit did not include the hiring date. <br /> The designated operator shall train facility employees for which he or she is responsible in the proper operation and <br /> maintenance of the UST system once every 12 months. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4.
