Site Health and Safety Plan <br />Petroleum Related UST Removal <br />L. GENERAL SAFE WORK PRACTICES. <br />1. All accidents and incidents must be reported to your supervisor immediately. <br />2. All defects/malfunctions which appear during the course of the work shift must be reported to the <br />supervisor. <br />3. No eating, drinking, smoking, chewing tobacco or gum, or applying cosmetics is allowed in the exclusion or <br />contamination reduction zones. <br />4. Employees shall be encouraged to inform their supervisor of any prescription medications or over the counter <br />medications they are using which may affect their abilities to work. <br />5. Employees shall not show up for work under the influence or in possession of alcohol or illicit drugs. <br />6. Only approved protective equipment shall be used by AES employees. <br />7. Employees shall not remove or disturb any covering, guards, or safety devices placed on vehicles, gears or other <br />moving equipment or machinery except to perform maintenance or repairs. Work on the equipment shall not <br />commence until the equipment has been deactivated, sources of energy removed, and the controls are locked and <br />tagged out. <br />8. Before starting any vehicle or machinery, or turning on electricity, gas, steam, or air, employees will check the entire <br />area to insure it is safe to proceed with the work. Out of service or locked out equipment is not to be started by <br />anyone, unless authorized by supervision. <br />9. Employees shall maintain good housekeeping of the facilities and remove or dispose of all unnecessary material. <br />10. Special operations including confined space entry, hot work, decommissioning of equipment for repairs require permits <br />to be signed by authorized personnel. A description of the procedures will be included in the enclosure section if <br />appropriate. <br />11. Trenching or excavations must be shored or sloped or appropriately prepared as required by OSHA standards. <br />M. SPECIFIC SITE SAFETY OFFICER RESPONSIBILITIES. <br />The Site Safety Officer or Designate will enter before any work begins and will verify that the established <br />zones are identified and escape routes are clear. <br />2. The daily duties will include the following: <br />a. Determine the wind direction and stay appraised of it throughout the stay. Identify the direction <br />during the tailgate safety meeting or informally with each affected employee. <br />b. Confirm the proper placement of emergency information and operational status of equipment and <br />the decontamination facility, <br />C. Monitor air as necessary for conditions that may cause injury or exposure and record all data. <br />d. Visually observe for signs of actual or potential life or health threatening hazards. <br />e. Note physical conditions of the site. Determine potential exposure pathways. <br />f. Use survey tape or markers to identify new boundaries of the zones. <br />g. Perform site safety inspections on all projects. <br />17 <br />
