The <br /> following are Class I violations: 108,109,110,111,112,118,119,120,301,302, 303, 304,305,311,317 <br /> 108. Secondary containment testing was last performed on 8/18/2008 and was due by 8/31/2011. <br /> Testing has not been completed and is 11 months past due. Secondary containment testing is required <br /> once every 36 months. Immediately schedule this test and provide 48 hours notification to the EHD. <br /> 109, 110, & 111. Annual leak detector testing, spill container testing, and monitoring system certification <br /> (including annual line test since there positive shutdown for the 91-octane and diesel UST systems had <br /> not been verified), were last performed on June 29, 2011 and was due by June 30, 2012. These tests <br /> are required once every 12 months. Annual leak detector testing, spill container testing, and monitoring <br /> system certification were conducted during this inspection today. <br /> 118. The 87 and 91-octane USTs, and the diesel UST are currently not being monitored by the existing <br /> INCON monitoring system as described on the monitoring plan. An amended operational permit was not <br /> obtained when the monitoring system was changed. The UST owner or operator shall notify the EHD <br /> within 30 days of any changes in the operation of the UST system. Immediately submit a permit <br /> application to the EHD for this change. An "Amended Permit Fee" applies. <br /> 119,120 & 317. The 91-octane and the diesel USTs have been found to be illegally abandoned since <br /> the two USTs were Red Tagged on July 20, 2011, and a Notice to Abate dated September 22, 2011 <br /> were served to you for illegal abandonment of the two UST systems. No person shall abandon an UST <br /> system without complying with the requirements of HSC 25298. Immediately obtain a permit from the <br /> EHD to remove this UST system, or a permit to temporarily close this UST system only if the USTs are <br /> to be reused within one year of closure. A "Failure to Obtain a Permit" penalty applies. <br /> 301. The 87 and 91-octane UST system, and the diesel UST systems are currently not being monitored <br /> to prevent unauthorized release. The underground storage tank system shall be operated to prevent <br /> unauthorized releases. Immediately begin operating UST system that will prevent unauthorized <br /> releases. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> Inspector:.
