Annular sensors sensors of the 87 and 91-octane USTs, and the diesel UST failed to detect <br /> a leak from primary to secondary containment and activate an audible and visual alarm and therefore, <br /> the two USTs are not monitored. The interstitial space shall be monitored with a monitoring system <br /> connected to an audible and visual alarm system. Immediately contact a properly licensed, trained, and <br /> certified contractor to verify if the monitoring system is capable of detecting a leak from primary to <br /> secondary containment and activating an audible and visual alarm under permit and inspection of the <br /> EHD. <br /> 305. All three STP sump liquid sensors and the three UDC sump sensors failed to stop the flow of <br /> product at the dispenser when tested. Facility also failed to stop the flow of product when fail-safe and <br /> sensor-out were tested. All monitoring equipment shall be maintained to activate an audible and visual <br /> alarm or stop the flow of product at the dispenser when it detects a leak. Immediately have a properly <br /> licensed, trained, and certified contractor to verify the sensors are capable of detecting a leak and <br /> activating an audible and visual alarm or stops the flow of product under EHD inspection. <br /> 311. The 87-octane line leak detector failed to detect a leak when tested. All line leak detectors shall be <br /> capable of detecting a 3-gallon per hour leak at 10 psi. The service technician adjusted the leak <br /> detector, retested it, and it passed. <br /> 311. The 91-octand and diesel line leak detectors failed to detect a leak when tested. All line leak <br /> detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. Immediately have a properly <br /> licensed, trained, and certified contractor repair or replace the failed leak detector (LG 113-x listed, if <br /> applicable) and obtain a permit within one business day from the EHD.
