It is EHD's understanding that no concrete will be broken for <br />these repairs. The permit has been approved with the following conditions: <br />1. Contact the local fire department and the San Joaquin Valley Air Pollution Control District for any applicable <br />permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval prior to commencing work. <br />All new addenda supersede the previous ones. <br />3. Contact John Alaniz at (209) 468-3147 to arrange the following appointments (a 48-hour notice is required for <br />inspection appointments). <br />• Secondary containment testing on all sumps where new penetration boots have been installed. <br />• If any primary piping should be undone, a primary piping test will need to occur. EHD will need to witness <br />the tightness test performed by SWRCB licensed tank tester or receive passing 0.1 gallon per hour test <br />results from the PLLD (if installed). A visual inspection is required for PLLD tested lines. <br />Please be aware that the facility cannot operate the UST system prior to the EHD inspection and/or approval. <br />4. Secondary containment testing for all sumps where penetration boots have been installation is required six <br />months after installation. Provide 48 hours notification to the EHD when this test is scheduled. <br />5. If concrete will be broken near the tank, a secondary containment test must be performed on the tank once <br />concrete is poured in accordance with the manufacturer's guidelines. <br />6. Any individual performing the work of a service technician must meet the following qualifications: <br />a. Possess or be employed by a person who possesses one of several permissible valid, current contractor's <br />licenses issued by the Contractors State License Board (may be issued to the installation company), or a <br />Tank Tester's license issued to the individual by the State Water Board, and <br />b. Possess or work under the direct and personal supervision of an individual physically present at the work <br />site who possesses a valid, current California UST Service Technician certificate issued by the International <br />Code Council (ICC), and <br />c. Possesses a valid, current certificate of training from the manufacturer(s) of the component(s) being tested, <br />repaired, or serviced. <br />7. Individuals installing underground storage tank system components must possess or work under the direct and <br />personal supervision of an individual physically present at the work site who possesses all of the following: <br />1868 E.
