You were required to submit this information by December 5, 2002. <br />Failure to correct the violations will result in continuing/additional violations. Failure to submit the completed <br />underground storage tank information by January 5, 2003 may result in further legal action. <br />The missing information include(s); <br />❑ Submit Current State Form A. <br />❑ Submit Current State Form B. <br />❑ Submit Monitoring and Response Plan. <br />❑ Submit Annual Line Tightness Test Results. <br />❑ Submit Certification of Financial Responsibility and <br />letter from the Chief Financial Officer. <br />❑ Submit Annual Line Leak Detector Results. <br />❑ Submit Annual Monitoring Certification. <br />❑ Submit Maintenance and Monitoring Log <br />® Permit submitted to repair / replace failed equipment. <br />❑ Submit Secondary Containment Test Result. <br />Most forms may be accessed from our web site address, �1 irw.gencitirs.corn%unitiii. <br />Leak detector of Regular Unleaded Fuel tank was tested and failed on November 5, 2002. Repair or replace the <br />failed leak detector under permit. An inspection is required to witness the functionality of the new or repaired leak <br />detector. A 48 hours notification to this office shall be given prior to inspection.
