Box 2009 <br />Stockton, California 95201 <br />RE: ADDITIONAL INFORMATION REQUESTED BY THE HEALTH DEPARTMENT <br />FOR APPROVAL OF PERMIT TO REMOVE <br />TWO UNDERGROUND STORAGE TANKS <br />PACIFIC BELL, LODI, CALIFORNIA <br />Dear Ms. Foley: <br />American Environmental Management Corporation (AEMC) has been contracted by <br />Pacific Bell (Pac Bell) to remove two underground storage tanks (USTs) at the Pac Bell <br />telephone switching facility located at 124 West Elm Street, Lodi, California. The original <br />application for permit to close these tanks was submitted by AEMC to the San Joaquin <br />Local Health Department (SJLHD) on 25 September 1990. An addendum to this <br />application, stating that AEMC would sample the soil beneath every 20 feet of piping and <br />including an Authorization To Release form signed by a Pac Bell representative, was <br />submitted to the SJLHD on 28 November 1990. <br />Per your telephone conversation of 3 December 1990 with Mr. David Oliver, AEMC is <br />providing the SJLHD with written confirmation that we will remove the piping leading to <br />the USTs, and send it and the USTs to Erickson's Inc. in Richmond, California for cleaning <br />and recycling into scrap metal. <br />Enclosed with this letter is a copy of the site Health and Safety Plan (not required on the <br />UST removal application), a triplicate copy of the UST Disposition Tracking Record <br />(originally enclosed with the initial application), and an updated State Contractors License <br />certification good through 31 October 1992.
