OPEN EXCAVATION HAZARDS <br /> The project involves the uncovering,excavation and removal of underground fuel <br /> tanks and pipelines. Primary us associated with excavation is exposure to <br /> the open excavation area,resulting in falls and or collapse. To mitigate this hazard, <br /> the following safety actions shall be takerL <br /> -No person shall be allowed to enter nay excavation greater then five feet deep <br /> or less than five feet when soil testing or-conditions indicated that the soils are <br /> Prone to movement. This SBP dose not address entry into excavation that require <br /> protection prior to entry. <br /> -All excavated soils(spoils)shall be placed a minimum of two feet back from the <br /> edge of the excavation and at a distance to prevent excessive loading on the face <br /> of the excavation. <br /> -The use of heavy and/or vibrating equipment(with the exception of the excavating <br /> equipment itself)adjacent to excavation shall be prohibited <br /> -The position and proximity of the back hoe and other equipment shall be considered <br /> when assessing the dangers of moving ground and the need to protect excavations <br /> form collapse. <br /> -All excavated areas shall be clearly marked and/or fenced to prevent unauthorized <br /> and/or accident entry into work areas. <br /> -Necessary foot and vehicle a shall be directed away form and around excavation <br /> work areas, and the routes clearly <br /> -Excavations shall be protected from intrusion of water. <br /> CO ED SPACE ENM <br /> The scope of work dose not require personnel to enter into tanks, excavations or other <br /> confined spaces. Field personnel are strictly forbidden to enter any confined spaces, <br /> for any reason, as a safety mandate of this SBP. therefore,the SHP dose not currently <br /> address confined space entry hazards and/or safety procedures. The SEP shall be revised, <br /> reviewed and approved prior to any confined space entry work, should this operation <br /> become necessary. <br /> Page 6 <br />
