A valid, current certificate of training from the manufacturer(s) of the component(s) being <br />tested, repaired, or serviced. <br />6. Qualified installers must possess or work under the direct and personal supervision of an <br />individual physically present at the work site who possesses all of the following: <br />a. A valid, current contractor's license issued by the Contractors State License Board, and <br />b. A valid, current certificate of training from the manufacturer(s) of the component(s) being <br />installed, and <br />c. A valid, current UST Installation/Retrofitting certificate issued by the International Code <br />Council (ICC). <br />7. The tank owner or operator shall log into the California Environmental Reporting System (CERS) <br />at http://cers.calepa.ca.gov/, update and submit the "UST Monitoring Plan", as appropriate. <br />8. A "UST Certification of Installation/Modification" form shall be submitted by the owner or the <br />owner's agent (CCR Section 2635(d)) before this upgrade is considered complete by EHD. <br />9. All components must be certified by an independent testing organization and labeled as required <br />by the organization (UL, ULC, ANSI, API, ASTM, etc.). See California Code of Regulations <br />(CCR) Title 23, Chapter 16, Table B for a complete list. <br />10. Permit expires 180 days from the date of approval.
