Annual monitoring system certification and leak detector testing were last performed on January 16, 2014 and was due by January 16, 2015. Testing has not been completed and is past due. These tests are required once every 12 months. Immediately schedule these tests and provide a 48 -hours notification to the EHD. Please be aware that a follow up inspection will be charged at our current hourly rate. This is a Class II violation. 112 HSC 25284.2 Failed to test spill containers annually. Annual spill container testing was last performed on January 16, 2014 and was due by January 16, 2015. Testing has not been completed and is past due at this time. This test is required once every 12 months. Immediately schedule this test and provide 48 hours notification to the EHD. Please be aware that a follow up inspection will be charged at our current hourly rate. This is a Class II violation. 114 CCR 2636(f) Failed to perform 0.1 gph annual line test for pressurized piping without fail safe or shut down. Based on the results of the most recent monitoring system certification, conducted on January 16, 2014, the diesel UST is not fail safe and does not meet the requirements of the annual line tightness testing exemption. Due to a malfunctioning diesel turbine, the failsafe operation of diesel UST could not be verified during the January 16, 2014 immediately. Immediately schedule this test to verify fail safe operation, and provide a 48-hour notice to the EHD. This is a minor violation. 204 CCR 2712(i) Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. This is a minor violation. 208 CCR 2715 Designated operator monthly inspection reports not maintained on site for at least 12 months. The January 2015 designated operator monthly inspection reports were not found on site, according to the site operator the facility has been in operation as of January 9, 2015. Designated operator monthly inspection reports for the previous twelve months shall be retained on site. Ensure that copies of the previous twelve months of designated operator monthly inspection reports are maintained on site. According to Mr. Singh (the site operator) no designated operator inspection was conducted in January of 2015. This is a minor violation. 209 CCR 2715(f) Designated operator employee training not performed or log not kept. According to Mr. Singh, the designated operator employee training was not performed for this facility. The designated operator shall train facility employees for which he or she is responsible in the proper operation and maintenance of the UST system once every 12 months. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD.
