The Bravo float -and -chain in the UDC 1/2 (91 -octane) sump failed to stop the flow of product at the dispenser when it was tested on January 16, 2014. All monitoring equipment shall be maintained to activate an audible and visual alarm or stop the flow of product at the dispenser when it detects a leak. A permit (SR0069897) was obtained for this repair, however, to date no EHD inspection to verify functionality has been conducted. Correct immediately by having a properly licensed, trained, and certified contractor replace the failed component with a functional component (LG 113-x listed, if applicable) under permit and inspection from the EHD. If the failed component can't be replaced immediately, there is a possibility that the 91 -octane UST system may be red tagged to prevent fuel inputs. This is a minor violation. 323 CCR 2636(f)(2) Leak detector failed to detect a 3.0 gph leak at 10 psi. The 87 -octane line leak detector failed to detect a leak when it was tested on January 16, 2014. A permit (SR0069897) was obtained for this repair, however no EHD inspection to verify functionality has been conducted to date. All line leak detectors shall be capable of detecting a 3 -gallon per hour leak at 10 psi. Immediately have a properly licensed, trained, and certified contractor repair or replace the failed leak detector (LG 113-x listed, if applicable) under permit and inspection from the EHD. If the failed leak detector can't be replaced immediately, there is a possibility that the 87 -octane UST system may be red tagged to prevent fuel inputs. This is a minor violation. Overall Inspection Comments: ONSITE TO CONDUCTA ROUTINE UNDERGROUND STORAGE TANK (UST) INSPECTION, TO DATE, NO ANNUAL MONITORING SYSTEM CERTIFICATION WAS CONDUCTED AT THIS FACILITY. PLEASE BE AWARE THAT THE FOLLOW UP INSPECTION TO OBSERVE THE MONITORING SYSTEM CERTIFICATION WILL BE BILLED AT EHD'S CURRENT HOURLY RATE. According to Mr. Avinash Singh, the site operator, to station was reopened after the remodel and began operation on January 9, 2015. Immediately schedule the required monitoring system certification and the secondary containment testing (which are both past due at this time), and provide the required 48-hour notification to the EHD. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by March 18, 2015. Consent to perform the inspection, take photos and collect samples was given by Mr. Avinash Singh, the site operator. This facility's designated operator is Christopher Bell ([CC expiration date: 12-02-2015) . Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement (if applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator Compliance.
