Since the Bravo box (float and chain assembly) in the UDC 1/2 was broken and not attached to the <br />float at this time, it would appear that the designated operator failed to visually inspect the UDCs. During <br />the monthly inspection, the designated operator shall visually inspect all spill containers and UDC <br />sumps, and any tank top sump that had an alarm for which there is no record of a service visit. Ensure <br />that designated operators performing monthly inspections at this facility are inspecting all of the required <br />areas and noting it on the reports. <br />204. The designated operator failed to check and note the correct test date for the monitoring system <br />certification on the February 15, 2013 designated operator monthly inspection report, the inspection <br />report listed the monitoring system certification date as July 21, 2012 (the monitoring system certification <br />was conducted on January 15, 2013). During the monthly inspection, the designated operator shall <br />check that all required testing and maintenance for the UST system have been completed and document <br />the dates they were done. Ensure that designated operators performing monthly inspections at this <br />facility are including all of the required information on the reports. <br />204. The designated operator failed to document all the alarms from the attached alarm history on the <br />April 4, 2013, May 17, 2013, June 13, 2013 designated operator monthly inspection reports and failed to <br />check that they were responded to appropriately. The missing alarms include the following: March 22, <br />2013 (1-2), May 17, 2013 (1-4) May 30, 2013 (1-1). During the monthly inspection, the designated operator <br />shall review the alarm history for the previous month, check that each alarm was documented and <br />responded to appropriately, and attach a copy of the alarm history with documentation taken in response <br />to any alarms to the monthly report. Ensure that designated operators performing monthly inspections <br />at this facility are including all of the required information on the reports. <br />303. The Bravo box (float and chain assembly associated with the 91 -octane product) in the UDC 1/2 is <br />broken and could not be repaired at the time of inspection and as such is not working properly to ensure <br />continuous monitoring of the tank system as required by CCR 2630(d). The technician onsite left the <br />aforementioned float and chain tripped ensuring the closed shear valve position. Correct immediately by <br />having a properly licensed, trained, and certified contractor replace the failed component with a <br />functional component (LG 113-x listed, if applicable) and obtain a permit within one business day from <br />the EHD, and retest the aforementioned component under the inspection from the EHD.
