Financial responsibility documents have not been submitted to the EHD. The last financial <br />responsibility certification in file is dated 2/11/11. Current financial responsibility documents are required <br />to be submitted annually. Complete and submit a copy of the financial responsibility for the missing <br />years immediately. <br />106, 202. Current approved copies of the monitoring and response plans were not found On site. The <br />monitoring and response plans must be current and approved by the EHD. Complete and submit a copy <br />of the monitoring and response plans for approval by the EHD within 30 days of receiving this report. An <br />approved copy will be returned for your records. <br />113. A new designated operator was hired in October, 2012 and another one was hired in December, <br />2012, and notification was not provided to the EHD within 30 days of the change. Any changes shall be <br />reported to the EHD with 30 days. Submit notification to the EHD identifying all the designated <br />operators for this facility immediately. <br />201. The alarm log for L5 - 1/26/12, L1 - 11/27/12, and L4 - 1/14/13 were not found on site. These <br />records shall be kept on site for at least 3 years. Immediately locate and maintain the alarm log for the <br />above listed alarms on site and submit copies to the EHD within 30 days of receiving this report. <br />204. Since the Chain was not attached on the shear valve, it seems that the designated operator failed <br />to visually inspect the UDC 3/4 during monthly inspections. During the monthly inspection, the <br />designated operator shall visually inspect all spill containers and UDC sumps, and any tank top sump <br />that had an alarm for which there is no record ora service visit. Ensure that designated operators <br />performing monthly inspections at this facility are inspecting all of the required areas and noting it on the <br />on the reports. C&P `f 1 a t <br />303. The chain on the Bravo Float and Chain was not connected thus is not working properly to ensure <br />continuous monitoring of the tank system as required by CCR 2630(d).
