The designated operator failed to check and note the correct test dates for the 2012, monitoring <br />certification and spill bucket testing on the December, 2012 designated operator r't�V�ly inspection <br />report. The date written is 1/28/12 and it should be 1/24/12. During the monthly inspection, the <br />designated operator shall check that all required testing and maintenance for the UST system have been <br />completed and document the dates they were done. Ensure that designated operators performing <br />monthly inspections at this facility are including all of the required information on the reporr'. <br />305. The diesel Bravo Float and Chain in the Dispenser #3/4UDC sump failed to stop the flow of product <br />at the dispenser when tested. All monitoring equipment shall be maintained to activate an audible and <br />visual alarm or stop the flow of product at the dispenser when it detects a leak. Correct immediately by <br />having a properly licensed, trained, and certified contractor replace the failed component with a <br />functional component (LG 11 3-x listed, if applicable) and obtain a permit within one business day from <br />the EHD. If the failed component can't be replaced immediately, there is a possibility that the Diesel <br />LIST system may be red tagged to prevent fuel inputs.
