The vent piping was <br /> observed as primary fiberglass with no secondary containment. <br /> This violation was written in the 4/3/2017 inspection report and has not been corrected. <br /> Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, <br /> update the required information, and submit for review by the EHD. <br /> This is a repeat violation, Class II. <br /> 105 CCR 2632(d)(1), 2634(d)Failed to have an approved UST monitoring plan. <br /> The monitoring plan is not current and not approved by the EHD. <br /> For all three tanks, under"training", NO is marked for <br /> "Personnel with UST Monitoring Responsibilities are Familiar with Training Documents." This statement needs to <br /> be marked YES as is required with DO training. <br /> This violation was written in the 4/3/2017 inspection report and has not been corrected. <br /> The monitoring plan must be uploaded to the California Environmental Reporting System(CERS). Immediately log <br /> into CERS, make the necessary changes, and submit for review by the EHD. <br /> This is a repeat violation, Class II. <br /> 113 CCR 2638(d)Failed to submit monitoring system certification test report within 30 days. <br /> Annual monitoring system certification, leak detector testing, and spill container testing was performed on 4/3/2017 <br /> and a test report was submitted on 1/19/2017 , approximatly 9 months late. A copy of the test report must be <br /> submitted within 30 days of the tests. Take all necessary precautions to ensure testing and submittal of test reports <br /> are performed in a timely manner. <br /> This is a Class II violation. <br /> 120 CCR 2635(d)Failed to submit UST Certification of Installation/Modification. <br /> The diesel underground fiberglass fitting located adjacent to under dispenser containment unit 3/4 was replaced <br /> under permit number SR0072496 and the UST Certification of Installation/Modification has not been submitted. The <br /> certification is required to be submitted within 30 days of completion of work to the California Environmental <br /> Reporting System(CERS).
