An inspections was performed on 1/29/2016 and a report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by within 30 days of the inspection. The following information has not been received resulting in a non-compliant status for this facility An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corrective actions taken or proposed is submitted within 30 days of receiving an inspection report. This is a repeat violation, Class II. 201 CCR 2712(b) Failure to maintain monitoring, maintenance, and follow up action records. Maintenance and monitoring records for the last three years were not found on site. The 4/3/2017 monitoring system certification test results were not found on site during the inpsection. On the 12/5/2017 monthly designated operator(DO)inspection report, Randall Kirby, DO, noted that"UDC 5/6 contain fluid and appears to be leaking gas. Dispenser has been shut down+shear valve have been tripped. Service work is scheduled for tomorrow." These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling;(2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment,ground water elevations, or other test results; (6)results of inventory readings and reconciliations. Immediately locate and maintain all maintenance and monitoring records for the last three years on site and submit copies to the EHD. This is a Class II violation. 204 CCR 2712(i)Current monitoring plan approved by the EHD not found on site. An approved copy of the monitoring plans were not available for inspection. A copy of these plans shall be accessible on site at all times. Immediately locate and retain a copy on site. The same violation was written during the 2/18/2015 routine inspection and the EHD never received a corrective action statement from EHD. This is a repeat violation, Class II.
