Lockheed Ct., Stockton IQA. <br />The permit to remove the existing piping system and install double wall fiberglass piping, <br />sumps and monitoring panel at the above referenced location has been approved wid) the <br />following conditions: <br />Removal Conditions: <br />1. Contact the fire department for their permit requirements. <br />2. Samples must be taken and analyzed as stated on pages 4 and 5 of the permit <br />application. <br />3. EHD must witness the soil sampling. Contact EHD to arrange this inspection <br />appointment. <br />4. Provide copies of hazardous waste disposal receipts. <br />5. Analytical results must be submitted to EHD directly from the laboratory. <br />Installation Conditions: <br />1. Contact the local fire department and San Joaquin Valley Air Pollution Control <br />District for applicable permits. <br />2. Ager Yanuary 1, 2005, an individual from the contracting company must possess <br />an International Code Council — "UST installation/retrofitting" certification and be <br />present on. the jobsite at all times. <br />2. A third party certified tank integrity test must be performed and results received <br />prior to scheduling the final inspection. <br />3. A third party certified piping line tightness test must be performed and results <br />prior to scheduling the final inspection. <br />6. All piping, connections, sumps and equipment shall bear a label/sticker from a <br />organization that adopt voluntary consensus standards, as submitted per plan/cut <br />sheets. <br />7. All piping must slope to a containment sump with an audible and visual alarm. <br />8. All equipment in contact with backfill must have corrosion protection. <br />9.
