This Report shall include <br />the following: <br />Chronological history of the incident; <br />Facts about the incident and when they became available; <br />Titles and names of personnel; <br />Actions: Decisions made and by whom; Order given to whom, and by <br />whom; and when; Action taken, who did what, when, where, and how; <br />Possible exposure of site personnel; <br />History of all injuries or illnesses during or as a result of the emergency. <br />These procedures are provided in addon to the HASP, prepared by Tait Environmental Systems <br />located at 701 N.
