Removed violation #113 and revised #326. This inspection report supersedes all previous reports.** Consent to perform the inspection, take photos and collect samples was given by Sukdev(Dave) Singh, manager. This facility's designated operator is Eric Meeker (ICC expiration date: 7/28/16) . The service technician was Felix Ramirez ([CC expiration date: 12/6/14). The manufacturer's certifications (Veeder-Root Level 4 and VMI Tester) were not observed on site and have not been verified. If these certifications cannot be verified, the testing may need to be redone. All EHD staff time associated with reinspections will be billed at the current hourly rate ($130). Note: A complete one hour hydrostatic test of the used oil spill container was not witnessed. The technician stated that the spill container was a pass. A measurement of 13 inches of water was taken after the water sat in the container for an hour, however, no measurement was taken at the beginning of the hour. This test will not be accepted by EHD and is considered incomplete. The retest of this spill container must be witnessed by EHD (a 48 hour notice is required for inspection appointments). All EHD staff time associated with this reinspection will be billed at the current hourly rate ($130). Note: The annual paperwork inspection was conducted on August8, 2013. Service request inspections after repairs at the site were conducted in October2013. Testing of the sensors and leak detectors was witnessed on October 11, 2013. Testing of the 89, 91, and used oil spill containers was witnessed on October 7, 2013. No violations were issued on these dates. Note: An approval for an emergency cold start of the monitoring panel (performed 11/25/13) was issued December 2, 2013. The inspection for this cold start was conducted on January 22, 2014, 51 days after the approval was issued. Inspections for emergency work must be scheduled within five days of approval. Please be aware that as of January 1, 2014, facility operators are required to upload the following UST program documents into the California Environmental Reporting System (CERS): UST Monitoring Site Plan, UST Certification of Financial Responsibility, UST Response Plan, UST Owner/Operator: Written Agreement(if applicable), UST Letter from Chief Financial Officer, and Owner Statement of Designated UST Operator Compliance. The monitoring system certification test results must submitted within 30 days of today's testing.
