UST Tank Information <br />forms for the 87, 89, and 91 tanks are not current in CERS. <br />87 Tank Form (TAO 189705): <br />Lists the primary piping as flexible; this piping is fiberglass. <br />Lists 'none' for the riser pipe primary containment; the riser is steel. <br />91 Tank Form (TA0189706): <br />Incorrectly listed as midgrade unleaded; this tank contains premium unleaded. <br />Does not list an overfill protection; the overfill prevention being used is fill tube shut off valve. <br />89 Tank Form (TA0189707): <br />Incorrectly listed as premium unleaded; this tank contains midgrade unleaded. <br />Any change of information must be updated in CERS within 30 days of the changes. Immediately log into CERS, <br />update the required information, and submit for review by the EHD. This is a repeat violation, Class II. <br />106 Monitoring and response plans not current or approved by the EHD. The monitoring plan is not current and/or not <br />approved by the EHD. The monitoring plans for all four tanks contained inaccurate information. <br />87, 89, and 91 Monitoring Plans: <br />The monitoring plan states that leak arms and failure/disconnect do not trigger pump shutdown; the monitoring panel <br />is programmed for pump shut down for either circumstance. <br />The leak detector MLLD manufacturer and model list two separate leak detectors; the leak detector being used is a <br />VMI 99-LD2000. <br />ELD testing is marked as a 'yes' under Periodic System Testing; this site was required to perform a one-time ELD <br />test and is not conducting periodic ELD testing. <br />The'second person having responsibility' was left blank. <br />Used Oil Monitoring Plan: <br />ELD testing is marked as a 'yes' under Periodic System Testing; this site was required to perform a one-time ELD <br />test and is not conducting periodic ELD testing. <br />The'second person having responsibility' was left blank. <br />The comments sections addressing the overfill prevention exemption lists "26:35-133"; the correct code section <br />should be listed as CCR 2635(B)(3). <br />The monitoring plan must be uploaded to the California Environmental Reporting System (CERS). Immediately log into <br />CERS, make the necessary changes, and submit for review by the EHD. This is a repeat violation, Class II. <br />112 Failed to test spill containers annually. Annual spill container testing for the 87 spill container was last performed on <br />August 8, 2013 and was completed on September 29, 2014, one month past due. This test is required once every 12 <br />months. Take all necessary precautions to ensure testing is performed in a timely manner. This is a repeat violation, <br />Class II. <br />113 Failed to submit monitoring system certification test report within 30 days. A monitoring system certification, leak <br />detector testing, and spill container testing were performed on August 8, 2014 and a test report has not been submitted <br />to the EHD. A copy of the test report must be submitted within 30 days of the test. Immediately log into the California <br />Environmental Reporting System (CERS) at http://cers.calepa.ca.gov and upload a copy of the test report as a <br />miscellaneous State -Required Document in the Underground Storage Tank section.
